In consideration of acceptance of my registration for participation in the martial art classes and performances related to said classes, I hereby, for myself, my dependent, executors and administrators, indemnify, hold harmless, waive and release any and all rights and claims which may result from this participation, against Victory Martial Arts Academy Inc., Phil Penticost (Owner/Head Instructor) ,any substitute teachers, representatives, successors, assigns, and volunteers for all injuries suffered by myself or my dependent(s) at any class, demonstration, tournament, or related martial arts activity. I have full knowledge that Martial Arts and related activities are contact sports and disciplines and that there is always, as in any contact sport and/or discipline, the possibility of injury. I am responsible for my medical costs resulting from any injury sustained while myself, and/or my dependent(s) is a student at Victory Martial Arts Academy, Inc. Therefore, I hereby assume the risk of any injury to myself, and/or my dependent(s) due to participation in the school operated by the above corporation. I am doing this of my own free will and under no duress
